Can you send intake forms to a new Member?

When a new member signs up, do you ask them to complete a form, i.e. PAR-Q or Medical form? If so, you can create the form and include it in their 'New membership confirmation email' that they receive when they sign up!

The first step here will be to create your intake form template by going to Settings > Client & memberships > Intake form. You can select here the “+new section” button, which will redirect you to the intake form details page. For the creation of the intake form, you can follow the regular intake form creation steps > Intake form.

When creating this, you will see the membership option in the Offerings section >

This option allows you to connect memberships enabled on your site to receive the intake form template you are implementing. If you want to enable your membership, this can be done in the Membership section in your Settings [See Guide].

Once this is connected, any new members will receive the option to fill out the intake form in their membership confirmation email >

All completed intake forms will then be saved on the client's profile, and the order associated with set-up fees of a membership, where you will be able to view the information if needed - see below.

If you have activated any deletion rules for your intake forms, this will also apply to your membership intake forms. To know more about intake form deletion rules, please refer to this guide.
