How to view a Client's Intake/Medical forms?
Depending on what a customer has booked in for they may be required to fill out an intake form prior to their visit.
In Trybe you can send these to customers via the Confirmation email for them to complete and submit the form online. No need for paper copies!
You can view the Intake forms guide to see how to create and edit intake forms for your different offerings i.e. Appointments / Fitness classes (sessions) etc.
The confirmation email that is sent out to guests for their order will include a 'Fill out intake form' button which will take them to the page where they can complete and submit the form.
Once a form is submitted, you can then view the intake form by heading to the Order > 'Intake forms' tab > select 'View' > you will then be prompted to enter in your password to ensure that the user has the correct User permissions to be able to view this confidential information**.
Instead of going into each individual order you can also view a client's intake form collection on their Client profile. To do this head to the Clients tab > select the client > scroll to the 'Intake forms' section.
Here each intake form listed will include:
The Order Reference of the Order the intake form was completed for
Date and time the intake form was filled out
You can then select on the intake form here to view it too! You will be prompted to put in your password again here.
The intake forms will be displayed in order of newest to oldest with the newest form that has been filled out will be at the top of the list.
Want to see all intake forms for a particular date?
You can gather all intake forms from one particular day via the 'Intake forms' report on the calendar!
Head to Calendar > Reports > Intake forms. (View second screenshot)
This will show up a list of all of the completed intake forms for orders on this date. It will list the guest's name, the order ref and the date/ time the form was submitted.
You can select on any of the forms to view them. If you press 'Download', this will place all of the intake forms into a PDF for you!
